Collection
of Information
We
may collect personal information, like names, postal addresses, email
addresses, mobile phone numbers etc. that are volunteered by you if you are
required to register or by volunteering for messaging services. Upon this
registration you are given a unique RxEOB ID, and you are no longer anonymous
to us. Once you are registered you are granted access to features of the RxEOB application.
This information is used to fulfill your specific requests unless you authorize
us to use it in another manner, for example to report an issue or discrepancy
with our application or services.
Tracking
Technologies
Technologies
such as: cookies, tags, and scripts are used by RxEOB and our partners,
Pharmacy Benefit Sponsors, affiliates, for analytics. These technologies are
used in analyzing trends, administering the application, tracking users’
movements around the application, and to gather demographic information about
our user base as a whole. We may receive reports based on the use of these
technologies by these companies on an individual as well as aggregated basis.
We
use cookies to remember users’ settings. Users can control the use of cookies
at the individual browser level. If you reject cookies, you may still use our application,
but your ability to use some features or areas of our application may be
limited.
As
is true of most applications, we gather certain information automatically and
store it in log files. This information may include internet protocol
(IP) addresses, browser type, internet service provider (ISP), referring/exit
pages, operating system, type of device used, date/time stamp, and/or
clickstream data.
We
may combine this automatically collected log information with other information
we collect about you. We do this to improve the services we offer you, to
improve marketing, analytics, or application functionality.
RxEOB
does not display advertising on our application or allow advertising with other
applications. RxEOB does not share any tracking with third party advertisers,
nor do our applications respond to DNT browser signals.
Distribution
of Information
RxEOB
will only share personal information with your health plan, governmental
agencies, or other companies assisting us in fraud prevention or investigation.
We may do so when:
(1)
permitted or required by law such as to comply with a subpoena.
(2)
trying to protect against or prevent actual or potential fraud or unauthorized
transactions; or,
(3)
investigating fraud which has already taken place and when we believe in good
faith that disclosure is necessary to protect our rights, protect your safety
or the safety of others, or respond to a government request.
RxEOB
will not share any personal data with third parties for marketing purposes. If
we are involved in a merger, acquisition, or sale of all or a portion of our
assets, you will be notified via email and/or a prominent notice on our application
of any change in ownership or uses of your personal information, as well as any
choices you may have regarding your personal information. RxEOB will not
disclose any of your personal information except when we have your permission
or under special circumstances described in more detail below.
When
using RxEOB application and sharing personal information, you will be sharing
that information with RxEOB and/or your benefit sponsor. You can choose not to
allow the transfer of your personal information by not using that particular
service.
In
the event of a promotion sponsored by other companies or cosponsored with
RxEOB, you will still have the choice of sharing your information and will be
notified during the time of data collection and decide to share or not to share
your data.
Please
be aware that approved applications that have links on our application may
collect personal information about you. The information practices of applications
linked to RxEOB are not covered by this privacy policy, and you must check
those privacy policies to verify how they may use your personal information.
We
may, however, share aggregate statistical information regarding our customers,
traffic patterns, and application usage with our partners, each of whom is
bound by their own privacy policies and applicable laws. This information will
always be in aggregate and will not be individual and personal in nature. Your
RxEOB account can be deleted or deactivated but doing so will result in
termination of access. Please contact RxEOB for further instructions about
deleting or deactivating your RxEOB account. Residual information will remain
within our archive records.
We
will retain your information for as long as your account is active or as needed
to provide you with services and use your information as necessary to comply
with our legal obligations, resolve disputes, and enforce our agreements.
Updating/Accessing/Amending/Correcting

your personal information changes, or if you no longer desire our service, you
may correct, update, amend, delete/remove, or deactivate it by making the

Testimonials/Comments/Reviews
We
post customer testimonials/comments/reviews on our application which may
contain personal information. We obtain customer’s consent to post their name
along with their testimonial via email prior to posting the testimonial. If you
wish to update or delete your testimonial, please contact us at
operations@rxeob.com.
Commitment
to Data Security
Your
personal information is kept secure. Only authorized employees, agents, and
contractors (who have agreed to keep information secure and confidential) have
access to this information. All emails and newsletters from this application
allow you to opt out of further mailings. Keeping your personal health-related
information private is vitally important to us. Knowing that you use certain
services or features may help us to offer you more relevant content; however as
with all personal information we do not make this available to any third
parties without your permission, except as required by law. Your RxEOB account
information is password-protected and encrypted with built in lockout features
to deter suspicious activity. Only you, your designated benefit enrollee, or
your benefit sponsor have access to this personal information. We strongly
recommend that you do not divulge your password to anyone. RxEOB will never ask
you for your password in an unsolicited phone call or in an unsolicited email.
Also remember to close the RxEOB application when not in use. This ensures that
others cannot access your personal information and correspondence.
Unfortunately,
no data transmission over the Internet can be guaranteed to be 100% secure. As
a result, while we strive to protect your personal information, RxEOB cannot
ensure or warrant the security of any information you transmit to us or from
our online products or services, and you do so at your own risk. Once we
receive your transmission, we will make reasonable efforts to ensure its
security on our systems.
Notification
of Changes
If
we decide to change our privacy policy, we will refer to those changes on our
Homepage prior to the change becoming effective, so our users are always aware
of what information we collect, how we use it, and under what circumstances, if
any, we disclose it. If at any point we decide to make any material changes or
use personal information in a manner different from that stated at the time it
was collected, we will notify users by way of an email prior to the change
becoming effective and wait for your consent. As mentioned before, users will
have the choice as to whether or not we use their information in a different

RxEOB

purpose, we enter into a contract that describes the purpose and requires the
recipient to both keep that personal information confidential and not use it
for any purpose except performing the contract.

Consumer Request”
To

information. You may also make a verifiable consumer request on behalf of your
minor child.

do not charge a fee to process or respond to your verifiable consumer request

that the request warrants a fee, we will tell you why we made that decision and

Information Sales Opt-Out and Opt-In Rights
RxEOB
does not sell your personal information. If this should change in the
future, you will be notified and have the option to opt-in if you so desire by
the process as defined in such notification.
Non-Discrimination
